The FA is reviewing the details of Sheffield United’s investigation into the circumstances surrounding Maddy Cusack’s death at the request of her family, who maintain that the midfielder took her own life for reasons connected with the appointment of the club’s manager Jonathan Morgan.

Cusack, 27, died at her home in Horsley, Derbyshire, on September 20. The cause of death has not yet been disclosed, but police did not treat it as suspicious. Cusack’s family are awaiting the results of the inquest into her death, which was opened and adjourned at Derby coroner’s court in October. According to The Athletic, Cusack’s family think that she died by suicide.

The family are unhappy with the results of an investigation, conducted by an independent third party
